CO 252 rejection code – what information are they lacking?

Hi everybody!

This is the first time I’m writing here. I have a strange claim that was denied with CO 252 code and the appeal wasn’t successful either. The clinical was attached but they still say that after consideration they don’t think that the visit is as complex as they need for 99205 (new patient). Here are they ICD-10s that were billed accordingly:
R10.84 Generalized abdominal pain
R11.2 Nausea with vomiting, unspecified
F41.9 Anxiety disorder, unspecified
F41.0 Panic disorder without agoraphobia

What am I doing wrong?

Getting history information

When reviewing a patients family and social history and there is no qualifying or pertinent information related to the HPI or chief complaint, but it is documented by the provider that they were discussed, if all other areas of comprehensive history are discussed, will those qualify the PFSH as complete, allowing for a comprehensive history?

A/P- With little information

Hi all, I wanted to get your feedback.

I have a provider that is noting an extended HPI, and Comprehensive exam, then under Assessment and Plan, just stating the diagnosis with the ICD-10 code.

For example, if the doctor is seeing the patient for Sinusitis or dizziness the assessment states:

J32.2 Chronic Sinisitis

R42 dizziness

Sometimes there is nothing else, no notes, orders or expansion in the note anywhere.

Very occasionally there may be an order for an xray or MRI.

I’m not seeing a true assessment and plan, giving their findings on how they got to the diagnosis. There is a disagreement going on that noting the diagnosis that way should count for a point- because it’s noted. I feel like shouldn’t they expand on it in some fashion? Showing their medical decision making? Impressions to score the risk?

Anyone had a provider like this in the past at all and have suggestions?

Pathology Coding / CLINICAL INFORMATION

Can anyone help me with regards to the CLINICAL INFORMATION on a pathology report? From my understanding we are to code based on the diagnosis by the pathologist. If I need to query, query but with regards to the clinical information; can in some instances, in particular, STOOL testing (diarrhea R19.7 clinical info) , CYTOLOGY. Can the CLINICAL INFORMATION be used as the primary diagnosis?

Hospital consult patient refused to give contact information

One of our doctors saw a self pay patient in the hospital for a consult. However, the patient refused to give any contact information to the hospital. I have no way to contact the PT or bill them, if we are not able to send statements or contact the PT are we able to send this patient to collections? This is a first for me and I want to make sure I have the correct information prior to telling the Dr we need to write this amount off. I appreciate any help :)
