52 years old Established Patient come for Gynecological Exam
HPI: Patient doing well except she is getting some flushes and flashes on the lower dose of estrogen
…..
…..

& for A/P : Well woman exam, Menopausal woman, On HRT with now some symptoms, once to evaluate the doses
"Discussed options with hormone therapy versus decreasing dose versus nothing versus how long she can take it and risks. I discussed the risks, benefits and alternatives to hormone replacement therapy with Heather. We discussed the results of the Women’s Health Initiative and the guidelines for HRT from ACOG. Some of the risks include stroke, clot, dvt, PE and she understands these risks and wants to take HRT". And Medication ordered: progesterone (PROMETRIUM) 200 mg capsule, Estradiol (DIVIGEL) 0.5 mg/0.5 gram (0.1 %) Gel in Packet.

Can we code extra 99213 with 99396?
Hormones review is included annually WWE?

Medicare Annual Wellness Visit and additional Well Woman Exam

Hello, fellow coders:

One of my providers performed an AWV and a separate well woman exam on two different dates of service. Since both were preventive, triggering use of G0468 (we’re an FQHC), Medicare paid the first well woman visit, but denied the AWV. My billing manager wants the provider to change the CPT code to an office visit (G0467) to get it paid, but the provider coded both correctly as preventive, with SOAP notes corroborating the coding. Has anyone had any luck in getting both an AWV and a well woman visit paid by Medicare?

Patient comes to gynaecologist for well womens exam but declines vaginal, cervix and uterus examination (breast where examined). can i still code diagnosis Z01.419(Encounter for gynecological examination (general) (routine) without abnormal findings)? or do i use Z00.00(Encounter for general adult medical examination without abnormal findings)?

I work for family practice docs and sometimes the patient is coming in for a pap/well woman exam. Commercial insurance. Should I be billing a 9939x with the Q0091? Because what if the patient decides to have an actual annual wellness visit a few months down the road? I would think it would get denied.

Diagnosis for well woman exam with OCP prescription

If a patient is seen for well woman exam with normal findings and is also given a 1-year prescription for her birth control pills, should I code Z01.419 and Z30.41? Or is it enough to code only Z01.419? The birth control surveillance isn’t listed by the provider as a diagnosis, but there was a prescription ordered in the system.

ICD10 coding for woman when male infertility is the cause of treatment

Hi all,

I’m hoping for some input on coding ICD10 for a woman receiving fertility treatment – IUI – when the male is infertile. Our providers keep using N46.9 – Male infertility for the woman, but, of course, it rejects as gender mismatch. Would Z31.81 (Encounter for male factor infertility in female patient) be an appropriate billable code for this situation? Any other input?

Billing for pap only on a later date from “well woman” exam

Hello,

One of our providers has asked me a question about properly coding well woman visits when the pap, (for whatever reason) occurs on a later date, separate from the initial exam. Technically, the pap should be included in the well woman exam, however, since it is being performed on a later date, we are having trouble deciding how to code it so the patient does not get billed for an uncovered well woman exam because it would fall inside the same year, which is generally all that is allowed. I hope this makes sense. I also think that it will be payer specific, but if I can have a general answer to start with, that would help a lot. Please let me know if I need to clarify anything. Thanks!!
