Open Sling CPT Code?

I just finished a practice exam from AAPC, and I’d love more clarification on the correct answer.

67 year old female having urinary incontinence with intrinsic sphincter deficiency is having a cystoscopy performed with a placement of sling. An incision was made over the mid urethra dissected laterally to urethropelvic ligament. Cystoscopy revealed no penetration of the bladder. The edges of the sling were weaves around the junction of the urethra and brought up to the suprapubic incision. A hemostat was then placed between sling and the urethra, ensuring no tension.

No where in the description does it say “open sling”. I’m not sure what it means by open, why is the correct code 57288?

Open patellar debridement

Hi, we billed this as 27599 compared to 27380. Insurance reviewed the record and said it wasn’t supported. I haven’t found a better code for the open patellar debridement; that is how the provider referred to it, although I could also see it as a tenotomy because he states he made vertical slits in the tendon. I also looked at 27306 but that is percutaneous. Thoughts?

Dx: Patellar tendinitis and chondromalacia patella

"I made a midline incision beginning at the inferior pole of the patella and extending distally to the tibial tubercle. I developed a plane between the paratenon and the subcutaneous tissues and then elevated the paratenon from medial over to lateral off the patellar tendon and excised it. I made some vertical slits within the patellar tendon."

He then follows with his notes on arthroscopic shaving of patella and lateral tibial plateau that we billed as 29877 and this has paid.

Lap appy converted to open w/ open hemicolectomy

*** Couldn’t delete *** I think it’s bundled since it went from lap to open where if it stayed lap, they both would have been billable?

Pt came in w/ severe acute appy w/ peritonitis. Went in lap but had to convert to open – had to do a RT hemicolectomy due to abscess.

Path confirms this.

What do I use for the appy code?
