Global???? not paying for exam with procedure

Hi All,

I am having an issue with insurances, mostly medicare not paying for an exam when a patient comes in for 97597. We have patients that develop new ulcers and the doctor has to check for other areas developing. Now they say it is global

AND now when patient comes in weekly for ulcer check and we charge out an office visit they are denying for global

Anyone have any ideas

MI Auto Carrier Paying Less Than Billed Amount

I am looking for information regarding Michigan Auto Claims and how to handle the remaining balance.
Some of our claims are paid less than the billed amount with the carrier stating it is "above the usual and customary" for our area.
We do not have a contract with the auto carrier and feel that they do not have the right to pay less than our fee.
Normally, when dealing with an insurance company that we have no contract with, the unpaid portion is billed to the patient.
The auto carrier is informing me and the patient that we are not allowed to balance bill the patient for the remaining balance.
I would appreciate any input on this issue.

Rebilling Higher Paying Revenue Codes

Hello Billing World – We have seen higher reimbursements for specific payers using different revenue codes, that are more applicable to our practice. We are considering rebilling previously processed claims, but we are apprehensive. We do not want to get flagged for upcoding, but we are unclear if rebilling with different revenue code is considered upcoding?

Any thoughts fellow billers?
