Florida Physician reports a HIPAA Breach that could affect over 60K patients

Authorities at doctor administrations merchant EmCare in Florida have advised 60,000 patients, representatives and contractual workers about a cybersecurity occurrence that may have uncovered their own data. An unapproved individual had the capacity to get to various worker email accounts that contained clinical information and statistic data, alongside driver’s permit and Social Security quantities of certain people, as indicated by an examination.

Patient Left Without Seeing Physician

If a patient is triage by the nurse but does not stay to see the physician nor receives labs, etc. can we bill for an ED Level 1 visit? I am getting conflicting information from in-house coders and our outside coding vendor. Any guidance you can provide is appreciated. Also, if you have a link to what is correct either way, even more appreciated.

Stuart Physician Convicted of 23 Counts of Health Care Fraud

Sheetal Kanar Kumar, M.D., 48, in the past of Stuart, Florida, was discovered blameworthy by a jury of submitting twenty-three tallies of social insurance misrepresentation (Case No. 18-14063-CR-Marra). Dr. Kumar is planned to be condemned on April 19, 2019 at 10:00 a.m., by U.S. Area Judge Kenneth A. Marra in Fort Pierce.

Choosing diagnosis codes for PT/OT/ST? – physician referral vs therapist’s evaluation

I work for a therapy company that provides services in various settings (via contracts) and am working toward my CPC certification (been in billing for years all over the specialty spectrum). I notice that some locations code encounters strictly based off of the physician’s referral. Is this an accurate practice? In reviewing the orders versus the therapist’s evaluations, I see a difference quite often as there is usually a medical diagnosis and then also a treatment diagnosis. I guess you could say I’m thoroughly confused at how therapy encounters are supposed to be coded because of what I am seeing compared to what I am studying in the books and guidelines.

Any advice or guidance would be much appreciated!

Physician orders for laboratory tests

Clinic is stating they placed an order for urinalysis ‘digitally’ so they don’t need to submit documentation for the UA. I’m unable to code it as there is no diagnosis or intent for the procedure indicated. What are the documentation requirements for a physician ordered laboratory test? Shouldn’t there be a document in the medical record that corresponds to the ‘digital’ order?
