Hello all, I have a Medicaid acct, Patient came in for OB Clinic Visit and starting having contractions, patient was sent to OB Triage to be check out and later sent home. Can I add a Modifier 27 for the Clinic Visit and a Modifier 25 for the OB Triage?

Medicare and coding coronary same branch modifiers
If I remember correctly, Medicare does not pay a branch of the same vessel. For ex. stent to the mid RC and stent to the posterior descending. The patient has Medicare primary and AARP secondary. Is there a specific modifier that needs to be applied to the second stent?

Can you have 2 new visits on the same day? New preventive and low level new office visit? I have found documentation that states you can and others that state that the preventive would be new and the sick visit would be established. Any thoughts?

Can 2 different providers with different specialties (hand & ortho) bill for an E&M on the same day? They are the SAME tax ID.

Transitional Care Management & Evaluation and Managment code SAME DAY?
Hello, just curious to see if other primary care practices are seeing Transitional Care Management Codes being used the same day as an Eval & Management code? Say they address something separate from the hospitalization, and the documentation supports, I don’t see anything that says otherwise that the provider can’t charge out a separate E&M. Thanks!
ER visit for contractions admitted and delivered same day – Physician
I was following the practice of billing the ED visit 99283 POS 23 (ED) for a patient who presents in labor to the ED. If she delivers same day that code is under 59409 POS 21 (Inpt) and billed. I cannot find anything on this site in writing so I can verify/prove what is correct to the billing dept. They state POS should be 22 (Observation) on the ED code. I disagree.

Coding e/m visits in the same day with an admission as observation then as inpatient
A physician sees a patient in his office in the morning and then again early that afternoon at which he send the patient to the hospital in observation status. Later that day he visits the patient in the hospital and admits the patient as full inpatient. Ehat E/M codes are billed for this?
