prcoedure code 77077 joint survey, single view, 2 or more joints (specify)

We have providers that are using this x-ray exam and billing 77077-26 for reading of bilateral hands and also dropping the 77077-26 for the reading of bilateral feet. Is it allowable to bill both and specify hand/feet or should it only be billed once per day regardless of the 2 different sites of the exam? Any help on this one would be appreciated.

Single Pelvic Lymph Node Resection – Laparoscopic

Can anyone tell me how you would code for a single enlarged pelvic lymph node that was resected during a TLH/BSO (58571) due to severe endometriosis? I wanted to use 38570, but I am not sure that is correct. I am thinking I may need to use the unlisted code?

Single and double balloon enteroscopy

Good morning. Can the code range 44360 through 44378, small intestinal endoscopy, enteroscopy be reported for single and double balloon enteroscopy via the antegrade approach? I realize the retrograde approach would be reported with the unlisted procedure code. I am finding mixed information, some state unlisted procedure code and others refer to codes 44360-44378.

Billing CPT 82542 multiple times for single specimen

Hi there, any help/assistance on the below question will be grateful:

While processing Lab & Pathology services, I find the provider billing CPT "82542" with multi-units for only 1 specimen(Urine). Whereas, multiple drugs/metabolites analysis was done. So the question is:

Can we bill multiple units for single specimen? As the code description says "Column chromatography, includes mass spectrometry, if performed (eg, HPLC, LC, LC/MS, LC/MS-MS, GC, GC/MS-MS, GC/MS, HPLC/MS), non-drug analyte(s) not elsewhere specified, qualitative or quantitative, EACH SPECIMEN"

Coding Single Stage Revision left total knee arthroplasty

I am looking for some advice. One of our providers performed a single stage revision left total knee arthroplasty with polyethylene exchange, irrigation & debridement and implantation of antibiotic impregnated beads. We billed the revision as 27486 (Revision of total knee arthroplasty, 1 component). The insurance is stating that this needs to be billed as 27310 for Arthrotomy for infection and states that the poly exchange is incidental. The 27310 does not seem correct to me since one component was actually removed and replaced with a new one. Any advice?

Single Payer insurance

If the U.S. changes to a single payer/Medicare for all type of system, what will the effect be on billing and coding? I would think it would eliminate a lot of our jobs. If everyone has the same insurance with the same rules, I would think billing and coding would be a lot more straightforward, with few if any variations. not trying to get political, just curious.
