inpt chemo treatment

I’m newer to coding oncology, I was trained briefly before the last person covering the division left. If a patient is admitted to the hospital for chemo treatment and they did the treatment and the doctor noted that they completed their course of chemo treatment on a certain date…the primary diagnosis for those days of chemo treatment are Z51.11…NOW the patient stays in the hospital for monitoring and count recovery for several days after do I still code the Z51.11 as primary diagnosis for all those days even though she is no longer having chemo treatment but it’s related to that OR do I now just code for the cancer as primary and any additional diagnosis?

43270 for treatment of Barretts

We are using 43270 for the Halo ablation for Barretts. However, we are also now getting denials especially from Private carriers that this code is investigational for the treatment of Barretts.
I need to write an appeal letter stating that 43270 is not investigational and the denial is debatable. Can anyone give me some sources about this code to show it is not investigational
and fight this denial? Thank you

Diagnosis vs. treatment diagnosis

I work in a physical therapy office and we recently switched to a new EHR system that has two different spots for ICD 10 codes. One says "Diagnosis" and below that "Treatment Diagnosis" I know the treatment diagnosis would be what the therapist is working on ex. muscle weakess, pain, ect. And the Diagnosis, what the doctor has diagnosed the patient with. But I have never had to split them into different spots before and am wondering if it matters for billing purposes or if it doesnt make a difference if the same codes are in both?

Z01.818 for E/M visits prior to chemo treatment

We are trying to get more information on using Z01.818 for office visits for patients prior to chemo treatments as they evaluate the effects of chemo on the patient and whether they can continue with their treatment on that day or not. Of course, if no treatment is given, this code cannot be used. These office visits are usually a level 3 or 4, sometimes a 5. We know that the chemo admin code has a low level inherent E/M included. One of the descriptions we found for Z01.818 is "an encounter for examination prior to antineoplastic chemotherapy".

Does anyone use this code? Are there issues with denials and/or certain payers? What is the criteria you use to use this dx?

Question about MD provider specialty for SUD treatment centers.

This is sort of a weird question, but I did not know the answer.

In most states, you can run a free standing substance abuse facility with a medical director of any provider specialty. However the standards of care for both United Healthcare and Cigna state that initial psychiatric evaluations have to be done by a board certified psychiatrist or addictionologist. I have heard that Cigna also allows the initial psych evals can be done by a specialized psych ARNP.

So here are my questions. Would it be okay for a psych ARNP to provide the initial face to face psychiatric evaluation? If so, can they report to a medical director that has a scope of practice outside of Psychiatry or Addictionology?
