What modifier should I use??

I have two claims being billed to the same ins company (different patients), CPT codes 59425 and 59426, but they are being denied because the patients received an injection on the same date (using 96372 for the injection). I’ve tried sending in corrected claims using modifier 25 but the insurance company doesn’t like that. I know I need a modifier due to NCCI edits, would -59 be better in this case?
Any suggestions?? Thank you!

What modifers should I used

If someone can help me I would greatly appreciated it. My question is if a provider A admitted a new born and provider B discharge the new born on another day and provider A billed only for the admission and get paid for admission and discharge(insurance contract stating like that) and they denied provider B charges? Can I used a modifier for provider B and resubmit the claim?

Should / Can Coders Enter Charges ????

My Question for the forum is should coders be entering charges or is it the responsibility of the providers, nurses, and other clinical staff to enter their own charges for service?? Where would I be able to find a guideline that tells what the law is for this. My personal thought is that if I the coder enters a providers charge it is not verified by the provider we also have an issue of the providers do not document enough to pick a charge. any help would be greatly appreciated.
