Weight used for 99478-99480?

Hello all!

When billing for neonatal weight based services (99478-99480) what is the appropriate weight to go by? Dosing or actual weight? Sometimes it differs and could be either depending on what you go by. I was taught to go by dosing weight but now am being told to use actual weight. Is there anywhere that has that in writing? I don’t want to get audited and have it come back on me.

Multiple Pass-through drugs used during IOL surg – will Medicare pay

We are going to start using Dexycu (J1095) and Omidria (C9447) during IOL surgery (66984) done in an ASC, both of these drugs are approved for pass-through status with Medicare. Will Medicare apply any additional adjustments to J0195/C9447 when both drugs are used during the same surgery? Thank you

External Cause codes used for Dermatology

Hello,
I’m looking to understand why my new employer listed External Codes (S00.00XA) primary, (Z08) secondary and then (Z85.828), when a patient is coming in with new lesions and also for F/U for personal hx of malignant neoplasm of skin. I’m not understanding why the external code usage? I would only use the Z08/Z85.828 for the E/M charge line.
