Hi,

Is the work status external cause code required for correct coding even when an encounter does not specify work status, volunteer activity, student activity etc. etc. ? When such things are specified I code, however, for example, if a baby has a fall, would y99.9 be required for "correct coding".

Repair of external sphincter muscle

Looking for some CPT coding direction on the following:
Reexploration of the perineal wound with extensive washout with pulse lavage
Reconstruction of external sphincter muscle between 6:00 and 9:00 o’clock position
Additional reconstruction of the anal verge between 6:00 and 8:00 o’clock position

Condensed report: The previous wound VAC was removed. The wound was the cleansed out extensively with pulse lavage. Significant reduction of edema of the tissues expecially in the perianal area as noted. Close inspection revealed that the external sphincter muscle was transected completely between 6:00 and 9:00 o’clock position. The edges of muscle where debrided and the muscle was approximated with interrupted sutures… Additional sutures were placed between the skin and anal verge to complete repair the area between 6:00 and 8:00 o’clock position. A wound vac was applied.

Appreciate any suggestions for this patient who has been reexplored several times in the past couple weeks from a MVA.

External Neurostimlator

Hello everyone…new to ASC coding and there is some discussion about the appropriate coding for an external neurostimulator. The code assignment by some coders in our clinic is 63685 but I can’t find another code specific to an external neurostimulator. I am finding that the attachment to an external stimulator is considered part of the work required for 63650, the percutaneous implantation of electrode, and is not separately reported. Can anyone confirm this, or have resources to confirm?

External Cause codes used for Dermatology

Hello,
I’m looking to understand why my new employer listed External Codes (S00.00XA) primary, (Z08) secondary and then (Z85.828), when a patient is coming in with new lesions and also for F/U for personal hx of malignant neoplasm of skin. I’m not understanding why the external code usage? I would only use the Z08/Z85.828 for the E/M charge line.

Thoughts?

reconstruction of external ear after being bitten off by dog

Hi
We have a 10 yr old boy who had his ear basically bitten off. He was seen by a doctor who placed a permanent suture and sutured his ear to the skin on his head to allow healing of the ear injury. He was told to come back in 6 months to a year to remove the suture and do a reconstruction. The original doctor retired and now my doctor is seeing him.

My doctor states he would need to relieve the suture and then modify/reconstruct the ear without using any flaps or grafts. He would most likely be able to close any gaps with just sutures.

I know i need to get authorization from the insurance, that is why we don’t want to use an unlisted code. I don’t think this qualifies as complex repair because it is not an open wound.

My question is this- what code do i need to use without using an unlisted code? Would it be 69300 otoplasty? Any help would be greatly appreciated.

External Fixator 20693 with 20694 Same Session

Provider makes adjustments to external fixator (under anesthesia) 20693, fixes a separate fracture and at the end of the service he removes the external fixator 20694. I have run NCCI edits and these codes do not bundle. Can’t find a source that states these can’t be billed together in the same session. These appear to me as a family of codes? Would appreciate some feedback and sources to give my provider.
