Los Angeles Dental Practitioner Sentenced to Forty Months in Jail for Role in $3.8 Million Health Care Fraud Scheme

A LA, California-based dental practitioner was sentenced to forty months in jail last week for his role during a $ 3.8 million health care fraud scheme during which he charged various dental insurance carriers for crowns and fillings that were not ever actually provided to patients. Assistant Attorney General Brian A. Benczkowski of the Justice Department’s Criminal Division, U.S. attorney Nicola T. Hanna of the Central District of CA, Acting Assistant Director responsible for John P. Selleck of the FBI’s Washington, D.C. Field workplace and Assistant Director accountable for Paul D. Delacourt of the FBI’s LA Field workplace generated the announcement.

97110 by General Practitioner or Family medicine in office Visit

Hello,

Request to guide that, can we report 97110 (Therapeutic procedure, 1 or more areas, each 15 minutes; therapeutic exercises to develop strength and endurance, range of motion and flexibility) for below scenario ?

In the office visit of Worker’s Compensation of California by general practitioner or family medicine, provider documents below paragraph with the heading of therapeutic exercises.

THERAPEUTIC EXERCISES
The patient was personally trained in appropriate warm up, strengthening and stretching exercises. These rehabilitative exercises were reiterated, demonstrated and reinforced; while the patient actively participated. This is also intended to be performed at home on a daily basis. The rehabilitation routine decreases inflammation, increases flexibility and strength of the specific structures in the injury adjacent locale to include both agonist and antagonist muscle groups, intrinsic flexor, extensor and rotary muscles as well as supporting structures. The exercises were intended to increase strength and range of motion and decrease pain. The minimum time required for CPT 97110 was exceeded for this patient.

Billing for a Nurse Practitioner in Training?

Is it possible for a new Nurse Practitioner (not credentialed – still getting hours) to bill for exams/services under the supervision of another fully credentialed Nurse Practitioner? If so, what documentation would we need in the charting and how would this go out on claims?

Good morning,

Scenario: multi specialty practice, ONE tax ID.. Ortho and PM&R. When a patient sees an ortho, and then sees the PM&R within 3 years, the PM&R can bill a new patient visit because the PM&R is a different specialty, and vice versa.

Is a nurse practitioner considered a "different specialty" as well? Is she able to code her initial visits with patient who have already been seen by one of the orthos or PM&R in the last 3 years?

Nurs practitioner sent pt to ER/supervising doc admitted

WE have a patient that was seen at our Rural Health Clinic by a nurse practitioner who sent the pt to the ER. This was not technically a direct admit. Nurse practitioner is supervised by Dr. M. at rural health clinic. Later that day, ER doc calls Dr. M to admit the pt to the hospital. Naturally Dr. M’s charge for the admit is being denied. My question is … since Dr. M bills with a different tax id for hospital charges and technically he wasn’t the treating doc for the OV, do we have a reason to appeal the admit code? We started to just write it off, but feel this really should be considered.

Coding Modifiers for First Assist Nurse Practitioner

I am getting Medicare denials for the use of AS when filing claims for a NP that first assists for a Neurosurgeon. Denials state inaccurate or need for different Modifier. I researched and found some info to use AS and 80. This combination also got denied. I have gotten info from other billers that they have no problem getting paid with the use of only AS. Another stated she had to use AS and 81. Any comments from other coders on this?

Nurse practitioner and X-rays

I’m a new biller, working at a new urgent care clinic. We have 2 nurse practitioners who have been administering and reading x-rays. Our doctors who are supervising (but not actually in the building), our NPs, and our office manager all believe that the NPs are able to read the x-rays. However, insurance is denying payment, stating the x-ray codes are beyond the provider’s scope of practice. Any advice on a solution? Eventually, we will have a contract signed with an outside company who will be reading the x-rays. In this case, do we bill the x-ray code with TC modifier? (And the other company uses -26? or vice-versa?) What about a way to get our current x-rays paid for? Please provide quotes or links I can use for reference, if possible, so I can pass on the info. Thanks!

Nurse Practitioner Billing

I am relatively new to billing and am struggling with this issue.

I work in an integrated physician’s office; we primarily perform chiropractic services but also currently have a Family Practice D.O. who performs H&P, Hyalgan injections, tendon injections, TPI, NCV/EMG, DME, allergy testing and shots, etc.

Our D.O. is retiring and a Nurse Practitioner will be taking his place. She will have a collaborating physician who will not be working in our office. The office manager and I have no idea how to bill for her.

We have no idea whose NPI to use and where on the HCFA form, if SA modifier is needed, etc.

I apologize in advance for my utter ignorance as I am primarily a coder.
