Is there a diagnosis I could use for this visit?

I know to code symptoms when the doctor doesn’t state a diagnosis but in this case I don’t even think there are any symptoms to code. What would be an appropriate diagnosis code for this visit?

Patient is a 6 month old.

HPI
Mom concerned about possible allergies
Eczema was flaring recently, mom thought it was from being in grass
Would get big welts after being in grass, welts on areas that didn’t even touch the grass
Back to back colds, cough hasn’t cleared for past 3 months

Doctor’s impression was:
discussed normal viral exposure during daycare ages, is growing well, no other physical sx, will watch and call for new or worsening sx

Practice Assessment Exams CPC or CRC…are there any out there?

I took my CPC exam in November and my CRC in March I passed both with 82-84%, in my eyes not too bad. I just took a very short HCC assessment exam, 11 questions got 6 out of 11 right. Needless to say I am bummed, I want to improve my coding skills and want to practice so I do better next time. My biggest fear is the ole if you don’t use it you lose it!! I miss having my nose in my coding books both HCC and CPT coding. I am looking at the Practicode however I have heard it is flawed so waiting to see if it goes on sale as it did last year, don’t want to pay full price for a product that may not be completely up to par, sorry just my honest opinion.

I know it is hard for a new coder to break in to the field just want to find a way to keep the dust from settling on my skills that I have worked so hard to get.

is there a code for …

I’m trying to find a code for "radical supracolic omentectomy and resection of splenogastric and splenophrenic ligaments."
This was done in conjunction with an Omentectomy and placement of a peritoneal port. these were all done open. I’m thinking its included with the omentectomy or is an unspecified code but have no idea what the comparative would be. Here’s a summary of the operative procedure.

"Upon entering the abdomen, a 2 mm miliary spread of peritoneal
carcinomatosis was observed on most of the surfaces including the
anterior abdominal wall, the pelvic side walls, the bowel, the surface
of the liver and the right hemidiaphragm. The omentum had 5-6 mm
nodules on it, only the supracolic omentum was left. The splenogastric
and splenophrenic ligaments also had 5-6 mm nodules, so all of these
were removed. There was a 6 mm umbilical hernia that was removed and
the fascia was freshened up and the hernia was closed. A peritoneal
port was placed and irrigated and confirmed patent. At the end of the
operation, the patient received epinephrine and Robinul in order to
raise her blood pressure so that we could check for hemostasis. The
irrigation fluid was repeatedly clear even during the final irrigations
during the induced hypertension. The sponge and instrument counts were
correct at the end of the operation and there was no suspicion of
enterotomy and no suspicion of urologic injury."

Help billing for wax surveillance when there is no wax

Hello All!

Im in search of feedback on how to code a particular visit. A patient has no voiced pain issues just states that there is a lot of wax in ears and they have to wipe their ears out. A Provider examines the patients ears and documents that both ears are free of cerumen. The codes provided for billing are 99212 with Dx code Z01.10 (encounter exam of ears and hearing w/o abnormalities). Medicare denied this claim as non covered when considered routine. I am aware that Z01.10 would usually be billed with routine and well visits ;so how should this visit be coded? Does the procedure code need to be changed? Would we not be able to bill for this service?

is there a diagnosis code for this situation?

The patient came to ED with right upper extremity pain and numbness. Oatient was found to have a right brachial artery thrombus. Patient has a history of atrial fibrillation and has been taking Eliquis, but stopped one week ago due to a change in insurance.

There are codes for intentional and unintentional underdosing, but is there anything to use for discontinuation for financial reasons? Would Z91.14 work?

HOME HEALTH in ASSISTED LIVING – Any Home Health Coders out there?

I did not see a subject specifically for Home Health questions. I am trying to locate information regarding CMS guidelines in regards to a Home Health Agency rendering services to a patient who is a resident of a assisted living facility, and requires that of a skilled nurse for wound care.
The assisted living facility has a RN on site, but the Home Health Agency would like to have their SN see the patient for the wound care. Does anyone know of guidelines that identifies this particular situation?
Please do not refer me to the CMS manuals, as I am very familiar with them, and have read Chp 7 and 10 more than one. Sorry, I do not mean to sound harsh, I just really need a answer, if anyone knows.

Are There Medical Reasons For Your Being Overweight?

Being overweight is not always an indication that it’s because you are eating too much or you are not exercising enough. It can simply come down to a medical condition and the very first thing you need to do concerning your excessive weight is to make an appointment with your doctor for a consultation.

To begin with, it’s very important that all medical issues regarding be overweight be ruled out before you start any type of diet regime. Many medical conditions can cause obesity and one of the most common could be problems with your thyroid. It can make you either underweight or overweight if it is not functioning correctly. One of the problems with your thyroid could be a lack of iodine in your diet and this can be easy identified by a test your doctor will perform to see if you are suffering from any type of iodine deficiency.

Iodine deficiency along with your being overweight can be creating your feeling of being physically tired and mentally just out of sorts. You may find that your hair isn’t cooperating and your skin seems to be drier. You could find that you are more sensitive to temperatures either hot or cold. These are all in good indicators that you may be having a problem with your thyroid.

No doubt you’ve heard this before but you’re going to have to start to consider your diet and your exercise. These are not easy steps to take or you would undoubtedly have done so a long time ago. One of the major problems is that we really don’t realize how many unhealthy foods we are eating and it becomes a habit and something that we soon lose control of.

It’s important that you eat enough protein and not too many carbs. Sugars consumed in excess will turn into body fat quickly and this is something that you want to keep in control of.

Hi,

I’m hoping someone can assist with this issue. I have a provider that has gone to a skilled nursing facility to review a patients records, discuss patient care with the SNF, and download information from a BIPAP machine. Is there any billable charges in this scenario?

Also, is there any billable charges when the provider has a phone conversation/encounter with the SNF to discuss patient care?

If this is a billable situation. Can you please tell me what codes should be used and what documentation needs to be done. Our provider is not affiliated with the skilled nursing facility.
