Coding high risk paps

Hi everyone! Our lab received a kickback on a code used for a high risk pap smear we received – it was for +HPV and I used the code but per Medicare, they require a screening code as the first listed diagnosis – with that being said, in the ICD-9 world, we always used "V15.89" (or something similar to that, I had to cross walk it to remember) as our first listed code which now, though, is "Z77.9" – I would keep using that but "Z77.9" is the code I use for DES exposure so I’m not quite sure what to do – Any thoughts? (I have included the message we got from the kickback below)

As explained more fully in chapter 10, Medicare requires that all screening Pap tests, both
routine and high-risk, be billed with a screening code as the principal (first listed) diagnosis.

High dose Influenza vaccination

A high dose flu shot was given to a pt who is under 65 years old due to the patient is immunocompromised. Insurance has denied it as not eligible due to the patient’s age. Would this be appealable or they would be denied regardless? Does anyone have came across the same issue? Any feedback would help! Thank you!

Long time coder but first time coding OBGyn. Not sure what dx to use for global c section for high risk pregnancy. Pt had severe pre-clampsia. Should I use as the primary
dx O09.893 with Z37.0 as second dx or should I use O34.211 as primary dx?

Controlling High Blood Pressure HEDIS Measure


The Healthcare Effectiveness Data and Information Set (HEDIS®) review for Controlling High Blood Pressure (CBP) consists of a random selection of members 18-85 years of age with a confirmation of a hypertension diagnosis on or before June 30, 2015 and documentation of the most recent blood pressure reading(s) in 2015 after the diagnosis of hypertension.

Notation of adequate control of blood pressure during 2015 is based on the following criteria:

• Members 18-59 years of age whose BP was <140/90 mm Hg.
• Members 60-85 years of age with a diagnosis of diabetes whose BP was <140/90 mm Hg.
• Members 60-85 years of age without a diagnosis of diabetes whose BP was <150/90 mm Hg.

Documentation of a diagnosis of diabetes must be provided if applicable to the patient.

Notation indicating a diagnosis of hypertension is acceptable on the following documents (at an outpatient setting):

• Problem list
• Office or progress note
• Subjective, objective, assessment and plan (SOAP) note
• Encounter form
• Diagnostic report
• Hospital discharge summary

Documentation of hypertension must include one of the following notations:

• Elevated BP (↑BP)
• High BP (HBP)
• Hypertension
• HTN
• Borderline HTN
• Intermittent HTN
• History of HTN
• Hypertensive vascular disease (HVD)
• Hyperpiesia
• Hyperpiesis

The following statements are not sufficient to confirm a hypertension diagnosis:

• Rule out HTN
• Possible HTN
• White-coat HTN
• Questionable HTN
• Consistent with HTN

BP reading is not acceptable in the following settings:

• Acute inpatient stay or an emergency department visit
• Outpatient visit (diagnostic test or surgical procedure performed (e.g., colonoscopy, removal of a skin lesion))
• Same day of a major diagnostic or surgical procedure (e.g., EKG/ECG, stress test, IV contrast for a radiology procedure, endoscopy).
• Self reported by the member

Assessing the Impact of High Deductible Health Plans on Radiology Practices

Before the days of managed care, insurance plans were “indemnity coverage” that reimbursed patients for their out-of-pocket costs. Physicians billed the patients and got paid when the patients felt like making payment, usually only after the insurance company had reimbursed them.  Often, the insurance money went elsewhere in the patient’s budget and the physician waited for payment.  The not-so-good old days!  With the advent of managed care contracting where physicians were paid directly by the insurance company, patient balance collections mostly disappeared.  Today the pendulum is swinging back in the opposite direction, requiring practices to once again face the necessity to collect significant balances from patients.

Supervision of high risk pregnancy codes and sequencing with multiple gestation codes

Hi, my practice uses O09.89_ on 90% of their encounters (Supervision of other high risk pregnancy). I think this is a history code as opposed to a current pregnancy code. Is this the case?

Also, does a multiple gestation code need a O09 (Supervision of high risk pregnancy) code before it or can the O09 be excluded since multiple gestation is considered high risk on its own?
