Medicare denials for Anesthesia overnight date span

Hello Forum
We have denials from Medicare for anesthesia services that start on one date then goes overnight to the next date. Medicare is denying unprocessable MA130.
M52 missing/incomplete/invalid from date of service, M59: missing/incomplete/invalid to date of service.
They want a new claim. I cannot appeal, the claim gets dismissed. Our computer system has the fields for the start time, end time and the start date, end date. Is anyone else having problems with this issue?
If so, what are you doing to correct the issue?

It can be said that few people can resist a well stocked bakery. However, no matter how much it might have, and regardless of its acknowledgment to differing lifestyles and allergies, there can still be problems. One such problem is lack of proper labeling for expiration dates.

Less Confusion for Staff and Customers

Baked goods that are not labeled with the proper expiration date can lead to a lot of confusion. A customer might be in need of a cake for a birthday party, but if they do not know how long the cake will stay fresh, they will be less inclined to purchase one. The same can be said of bread and similar items, as well. Keeping the staff and anybody who visits the bakery properly informed on the lifespan of the food is a good way to stay in business.

Date coding can be crucial for present and future purchases. Somebody might take something home only to discover that it is long past its “best if used by” date. Billing for pap only on a later date from “well woman” exam

Hello,

One of our providers has asked me a question about properly coding well woman visits when the pap, (for whatever reason) occurs on a later date, separate from the initial exam. Technically, the pap should be included in the well woman exam, however, since it is being performed on a later date, we are having trouble deciding how to code it so the patient does not get billed for an uncovered well woman exam because it would fall inside the same year, which is generally all that is allowed. I hope this makes sense. I also think that it will be payer specific, but if I can have a general answer to start with, that would help a lot. Please let me know if I need to clarify anything. Keeping Your Radiology Practice Up to Date on Medicare Quality Reporting

Medicare-quality-reporting.pngThe Centers for Medicare and Medicaid Services (CMS) issued two reminders recently that physicians must be working constantly to maintain compliance with the Medicare quality reporting programs. The current regulations call for adjustment of the fees paid to physicians for services to Medicare patients based on annual measurement of the physicians’ performance under quality and cost metrics.  Radiologists must focus on their quality measures because the system assigns them to an Average Cost pool by default since they have little or no control over this factor.

Date of Service for billing G0180- Home Health Certification

I’m looking for the advice on what date of service should we use when billing for Home Health Certification (G0180)? Should we bill with the date of face to face encounter, the date when the certification was actually signed by provider or with the start date of home health service? I would bill with the date it’s signed by provider but in my office I was told to use the date of the face to face encounter. What is the right way to bill for it?
