Anesthesia Time on Claims Question

This Maybe an Odd Question:
I have been coding for anesthesia for almost 12 years and we always report the anesthesia start and stop times with total time in minutes noted on the claim.
We also report the minutes in the required field.

My question is: Is this necessary for payment? Are we wasting production time by always providing this information?

I have seen one source specifically state the start time and end time is to be reported on the claim.
However, all other sites I have scoured just say the CMS statement of how the time must be reported in actual minutes.

I only ask because we recently took over billing for a client and were reviewing older claims that nothing had the anesthesia times on, but they were paid!

This was shocking to me.

I am a graduate of the Medical Coding Specialist program at Madison College and went on to continue my education in the Advanced courses offered in Medical Coding. I am currently enrolled in the online internship Practicode with the American Academy of Professional Coders. Billing for time spent counseling for Morbid obesity when done during a Physical

Provider documents
Morbid obesity – E66.01, (HCC), 50 minutes spent with the patient, with over 50% of that spent on discussion of morbid obesity, her attempts at weight loss, the risks and benefits of bariatric surgery. Patient would like to proceed with evaluation.
This was outside of the patients AW visit of 30 minutes
The excludes note for 99404 indicates not allowed with her Physical 99395
The 50 minutes fall short of a prolonged service code
Would it be appropriate to add an E/M for weight loss counsel consideration for Bariatric surgery during her Wellness visit?

A little rusty, looking for a study teacher to help me pass the 1st time around test

I live in the Amherst, OH area and I am looking for someone to help me study with before signing up for the CPC coding exam. I want to pass it the first time around. I completed my online course with Penn Foster 3-4 months ago and I am a tad rusty. I did purchase the test questions through AAPC, but I am willing to pay if someone that has been in my shoes and is now a coder can help me out and be my study partner to help me pass the first time around would be awesome.

Auditor states drug ordered as IV over 2 minutes must have a doc stop time?

Hello everyone,

We are currently experiencing a situation in which an auditor is stating that all IV drugs ordered by the physician with an expected duration of 1-2 minutes must have a documented stop time in order to charge the 96374, 96375 or 96376.

I have never encountered a situation like this in over 5 years. Perhaps I’ve missed verbiage in the CPT coding book that states it must be present?

Any help would be appreciated.

Time doesn’t support level selected

Hello!

I have a question…. 99215 was billed (documentation supports this code), but only 25 minutes (>50% counseling was spent with the patient). I understand that a provider can select a higher level of service when time is documented… but can time spent lower the billed service to a 99214? Or does documentation trump time in this situation?

Prothombin Time Coding Requirements

What are the exact requirements for coding for prothombin time? Here is an example of information I was provided: "patient has to be discharged within 30 days of DOS, no provider visit same day and documentation has to say the provider asked the pharmacist to see them". I have researched online and cannot find any information that explains so in-depth, other than there cannot be a provider visit the same day. Can anyone please let me know the proper coding requirements for this service? Thank you.

Total Time allotments for CPT Codes (Pre, Intra, Post – Service time allotments)

Hello everyone,
I’m looking for an excel spreadsheet I can download with all the breakdowns for CPT services (i.e. pre/intra/post minutes for each CPT code). I believe this information came from CMS…? My brain is fried today…I know I have downloaded this information previously, but can’t recall where.
